About you

Tell us who you are and where you work.

A few optional questions to help us understand who we're serving and build more equitable programming. Skip anything you'd rather not answer.

Your family

Help us understand who will be joining you at Tales week.

Allergies, accessibility needs, medical considerations — anything that helps us support your children well.

Which track?

We have two ways to participate. Choose the one that fits your situation.

We will secure discounted Education Day Passes for scholarship recipients who need them.

Your story

No right answers here. Write as much or as little as feels right — just give us enough to understand your experience, your story, and your voice.

Question 1

Tell us about your life as a service industry parent. What does a typical week look like for you?

Paint us a picture. The logistics, the juggle, the moments that make it hard and the moments that make it worth it.

Question 2

What has being a parent cost you professionally — opportunities missed, events skipped, moments where you had to choose?

We know this industry asks a lot. We want to understand the specific ways parenthood has created barriers for you.

Question 3

What would full access to Tales of the Cocktail mean for your career right now?

Be specific. What sessions would you attend? What relationships would you build? What would change professionally if you could show up fully?

Question 4 — Optional

Is there anything about your caregiving situation you'd like us to know?

This may include kinship caregiving, single parenthood, foster care, or any other circumstances that feel relevant.
